Abstract: In the article, the authors present their study to assess the influence of the social determinants of health (SDOH) on the post-discharge mortality of veteran patients who received medical care in transitional care units. Also cited are the sociodemographic factors that could affect health like marital status, occupational class, and education, and the Results showing that veterans with more comorbidities and living in rural areas have higher risk of mortality.
