Creating and evaluating service navigation networks for the military–connected population

Abstract: The article discusses the challenges faced by military-connected individuals in navigating benefits and services, highlighting the need for collaboration between clinical and social service providers. The D'Aniello Institute for Veterans and Military Families at Syracuse University developed AmericaServes (AS), a model that creates coordinated networks of organizations to address the health and human service needs of this population. The AS model has operated in 18 regions, assisting over 43,000 clients with 106,000 requests, demonstrating effectiveness in care coordination through indicators of timeliness, appropriateness, and scalability. Ongoing research aims to measure the impact of AS exposure on health outcomes, benefits utilization, and costs by matching AS clients to VA patient data.
