Explosive ordnance disposal service members with traumatic brain injury demonstrate enhanced music skill acquisition in music therapy: Case series

Abstract: Traumatic brain injury (TBI) is a leading concern in the military health system, affecting rehabilitation and quality of life of military-connected populations. Innovative treatments, including creative arts therapies, have successfully used strengths-based approaches to address symptoms of TBI and motivate treatment engagement. Case studies presented in this article reflect clinical observations from music therapy (MT) sessions with active duty service members (SMs) across service branches, specifically those trained in Explosive Ordnance Disposal (EOD). These EOD-SMs have been diagnosed with mild TBI (mTBI) and present with post-concussive symptoms (PCS) that impact functioning in familial, social, and occupational domains, including cognitive impairment and emotional dysregulation. Additionally, some were diagnosed with post-traumatic stress disorder and presented with comorbidities including insomnia, hypervigilance, and depression. Music therapy with SMs focused on addressing PCS by implementing a clinical protocol using a music-making intervention on the piano. Explosive Ordnance Disposal-service members' performance and completion of the clinical protocol demonstrated a higher level of dexterity, memory retention and recall, and emotional regulation than non-EOD-SMs. Explosive Ordnance Disposal operations and music-making are sequenced cognitive and motor processes that require physical and mental agility and enhanced emotional resilience to perform under pressure. As such, EOD-SMs may develop abilities that promote acquiring basic music skills through pattern recognition and recall. These abilities may explain EOD-SM's observed higher performance in an MT clinical protocol than non-EOD-SMs with mTBI and PCS. We hypothesized that extensive professional training and experience in EOD or a similar military occupational specialty would accelerate SMs' music skill acquisition and learning.
